How can I monitor unauthorized grade changes?

If you have the "View Client Logs" permission, you can access the Client Logs page (via the Configure page). This page logs all attempts at changing academic info by people who aren't authorized to do so.

The Client Logs page shows the date and time of attempted changes, along with which user account made the attempt, and a detailed message explaining what that person tried to do. Use the "Filter" bar look for attempts made by a specific person, made on a specific date, etc.

The information shown on this page may be the result of teachers innocently mis-clicking. This is no problem!

This page can also be helpful for identifying less-innocent attempts, in case a teacher's account was compromised. Either way, Schoolrunner displays an error message informing users that they don't have the right access to make changes.

If the teacher truly should have access to edit results for a course, s/he will need to be made the teacher of record on at least one of that course's sections, or s/he will need the "Edit All Grades" permission.
