More Bubble Sheet Configurability

Several clients have asked for greater configurability with bubble sheet answer choices. The current functionality is:

• multiple choice questions default to a range of A-D if the correct answer is within that range
• multiple choice questions default to a range of A-[correct answer] if the correct answer is beyond the A-D range

We're planning to add an "Answer Choices" field to the "Input" tab, which will let users define a custom range of answers (A-G) or a custom list of answers (A, B, X). For any question with something in this field, we'll respect the range/list provided. For other questions, we'll default to the current behavior.
